Preparing For Your Exam

There are just a few steps that you should take prior to your visit to ensure peace of mind and to reduce personal expense.

Step 1: Know and understand the coverage provided by your insurance provider. Some providers require preauthorization before covering a test ordered by your physician at AMIC. You can find out if this is necessary by calling your insurance provider, informing them of what has been ordered and asking if and how you can obtain preauthorization.

*Reminder* Medicare and Medicaid require written orders for all exams.

Step 2: Review the test that your physician has ordered for you. Select from the list below to learn more about your specific exam.


Step 3: On the day of your appointment, make sure to bring your valid and current insurance card, name and address of your claims payer(if different from the insurer), your ID and group numbers, and the name of the policyholder or subscriber. If applicable, for workers' compensation, automobile or personal injury cases, we require the name and address of your attorney as well.

Questions about your bill?

Why wasn't my insurance carrier billed?
If your insurance information is in our billing system when the services are entered, your insurance carrier will be billed directly. However, if the information is inaccurate or incomplete, then we may be unable to bill your insurance carrier.

Why didn't my insurance carrier pay for the services?
To obtain a clear understanding of your benefits and limitations or your insurance coverage, it is best to contact your insurance carrier directly for an explanation of the way your claim was processed and/or denied.

Why am I getting a bill now, when services were provided so long ago?
Once a response isi received from your insurance carrier and we have confirmed that there is additional patient liability, we will bill the patient. However, this process is dependent on when we receive a response from the insurance carrier. In addition, insurance carriers often request additional information that could delay the processing of your claim.

Need a copy of your images?

We will be happy to make your images/films available for pick up at any of our imaging centers upon request. Simply call our Medical Records Deparment at (312) 807-3555 Ext: 2 at least 2 business days before you would like to pick up your films. For urgent and same day requests, please call Medical Records.
